Submission Guidlines

1. HOW TO ENTER 

  1. The Face of Laser Center of Maryland Contest (the “Contest”) will begin on Monday, May 5, 2008 at 12:01am Eastern Time (“ET”) and end on Sunday, August 3, 2008 at 11:59pm. ET (the “Contest Period”)
 
  1. To participate in the Contest, log onto www.1019litefm.com during the Contest Period, click on the “Laser Center of Maryland” Contest page, enter your first and last name, complete address, city, state, zip code, home telephone number, date of birth, a valid email address, as well as upload a picture of yourself [CONFIRM. IF PICTURE ID NECESSARY FOR ENTRY INSERT SPECIFICS IN REGARD TO PICTURE – WHAT DOES IT NEED TO BE OF, THE SIZE, FORMAT, ETC] and write an essay in a hundred (100) words or less, why you would like to win that month’s prize (the “Entry”) in the online entry form.  One (1) Internet entry per person and one (1) Internet entry per email address is permitted. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry.  The authorized account holder is the natural person who is assigned to the email address by an Internet access provider, online service provider or other organization that is responsible by assigning email addresses or the domain associated with the submitted email address.  All entries submitted become the sole property of WLIF (the “Station”) and will not be acknowledged or returned. For purposes of these Official Rules, “receipt” of an online entry will be solely determined when the www.1019litefm.com web server records the entry information.  Any automated computer receipt (such as one confirming delivery of email) does not constitute actual receipt of an entry form for purposes of these Official rules.  Use of any device to automate entry is prohibited.  The Station’s computer is the official time keeping device for this Contest.
 
  1. Entries must be received by [INSERT DATE AND TIME] to be eligible for the May drawing, [INSERT DATE AND TIME] to be eligible for the June drawing, and [INSERT DATE AND TIME] to be eligible for the July drawing.
 
  1. The Station is not responsible for entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties or other technological glitches.
 
  1. Limit one (1) entry per person, per month. Non-winners for any given month are eligible to re-enter for the following month’s Grand Prize.  Separate entries will be collected for each month’s Grand Prize drawing.
 
  1. Photographs can only show the Contestant.  Photographic entries that include persons other than or in addition to the Contestant will be disqualified.  Each photo entry must be the original creative photo work and/or property of the Contestant and must not contain copyrighted materials or any other materials, which may be restricted, prohibited, or outlawed by federal, state or local laws.  All photo entries and their content become the property of the Station and will not be returned.  The Station, at its sole discretion, reserves the right to disqualify and/or destroy any photo entry if such photo entry contains nudity or partial nudity, illegal, indecent, pornographic, sexually explicit content, or otherwise offensive material such as aberrational behavior, graphic violence or drug abuse, or if such photo entry contains material whose content is deemed at the sole discretion of the Station to be unsuitable for public viewing or posting via the Internet. By participating in the Contest, Contestants agree to comply with the Official Rules of the Contest and any rules or guidelines established by the Station or its representatives regarding entry, participation or the prize of this Contest.  Representatives of the Station  in their sole discretion, may disqualify any Contestant from participating in the Contest and will waive any chance the Contestant may have had of winning the Contest. 
 

2. ELIGIBILITY RESTRICTIONS 

  1. The Contest is open to all Station listeners who are 21 or older and who legally reside in the greater Baltimore metropolitan area. Employees of the Station, its licensee, its corporate parents, subsidiaries, and affiliates, its advertising agencies, participating sponsors/promotional partners, other radio stations in the Baltimore metropolitan area and the members of their immediate families and/or households are ineligible to participate or win. Subject to all applicable federal, state, and local laws and regulations.
 
  1. Listeners can win a prize only once every 30 days.  Listeners and members of their immediate family and/or household can win a prize valued at $600 or more only once every six months. Only one winner per household is permitted in any Station Contest
 
  1. Winners must hold a valid government issued ID and sign and return any required Affidavit of Eligibility and/or Publicity/Liability Release.
 
  1. Contestants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent.  The Station will disqualify any entry from individuals who do not meet the eligibility requirements, and the Station will also delete any entry received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
 

3 PRIZES 

  1. Three (3) Grand Prizes will be awarded, one (1) for each month of the Contest Period. Prizes are as follows. 
    • The Grand Prize for May is a Sclerotherapy treatment (Leg Vein Removal) at Laser Center  of Maryland, 484-A Ritchie Highway, Severna Park, MD  21146.  Consultation appointment must be made within thirty days of winning and procedure must be done within thirty days of that consultation.  Approximate ARV= $1,172.00 (depending on the number of treatments required). 
    • The Grand Prize for June is a Facial Rejuvenation at Laser Center  of Maryland, 484-A Ritchie Highway, Severna Park, MD  21146.  Consultation appointment must be made within thirty days of winning and procedure must be done within thirty days of that consultation.  Approximate ARV= $5995 (depending on the number of treatments required)
    • The Grand Prize for July is a Liquid Facial Enhancement at Laser Center  of Maryland, 484-A Ritchie Highway, Severna Park, MD  21146.  Consultation appointment must be made within thirty days of winning and procedure must be done within thirty days of that consultation.  Approximate ARV between $545 to $2,500 depending on what procedure the winner needs and the number of treatments required
    • In addition to the specified treatment, each winner will be featured in a brief video to be shown on the Laser Center of Maryland website and potentially on the Laser Center of Maryland radio commercial.  Winners will not be compensated for their appearances.
 
  1. Winners will be subject to the terms and conditions set forth by the provider of the certificates.  Winner is solely responsible for all applicable taxes, post-procedure care and any other expenses not offered specifically herein.  The winner will be subject to approval for the service by the doctors/facilities providing the service. The participating doctors/facilities have the right to refuse to perform the service at their sole discretion, based on medical reasons, patient’s unrealistic expectations from the service or any other reason they see fit to disqualify the winner.  Prior to awarding prize or prize certificate, the winners must visit the Laser Center of Maryland for a free consultation to determine their viability for receipt of the service they won.  If, upon examination, Laser Center of Maryland determines that, for whatever reason and in its sole discretion, the winner is not eligible to receive the service, the prize will become null and void.  In such event, the radio station has the option to randomly select another qualifier to receive the Grand Prize.  No substitute prize will be made available to the original winner.
 
  1. All prizes or prize certificates may be picked up at the office of the Station at the address below.  The winner will forfeit any prize or prize certificate not claimed thirty (30) days of winning. 
 
  1. There is no substitution, transfer, or cash equivalent for prizes, except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
 

4. WINNER SELECTION AND NOTIFICATION 

  1. Decisions of the Station management with respect to the Contest are final.
 
  1. The essays and photographs received as part of each timely-submitted Entry will be posted on the station website at www.1019litefm.com.  Website visitors will be invited to vote on the Entry that they believe is the most deserving to receive the Grand Prize.  The winner for each month will be the person who submits the Entry that receives the most online votes by the following deadlines: 
 
 
  1. Odds of winning depend on the number of eligible entries received.
 
  1. That person need not be listening and need not call in to win. 
 
  1. Each Grand Prize Winner will be notified on via email or phone. If winner does not return the notification attempt within five business days, prize will be forfeited and an alternate winner will be selected in a random drawing of al eligible entries received]. Winners must execute and return any required Affidavit of Eligibility/Release of Liability Prize Acceptance Form within ten (10) business days of notification attempt or prize will be forfeited and an alternate winner may be randomly chosen.
 

5. CONDITIONS 

  1. Payments of all federal, state and local taxes are solely the responsibility of the winners.  For any prize valued at $600 or more, Winners will be required to sign an IRS Form W-9 or the equivalent, or prize will be forfeited.
 
  1. By participating in the Contest, the winners agree to have their Entries, names, voices, or likenesses used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize. 
 
  1. The Station, and Laser Center of Maryland, will each require Contest winners to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.
 
  1. The Station, in is sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the Station Web Site or is otherwise in violation of the rules.  It further reserves the right to cancel, terminate or modify the Contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
 
  1. The Station reserves the right to make changes in the rules of the Contest, including the substitution of a prize or equivalent value, which will become effective upon announcement.  If due to circumstances beyond the control of the Station, any prize-related event is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
 
  1. Failure to comply with the Contest rules may result in a Contestant’s disqualification solely at the discretion of the Station.
 
  1. The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the Contest, or in the announcement of a prize.
 
  1. Copies of the written Contest rules and a list of winners (when complete) are available during regular business hours at the main studio of WLIF, 1423 Clarkview Road, Baltimore, Maryland 21209.
